THE BALANCE MASTER:
    Physmed will provide our patients with a Balance Master. The Balance Master is a state of the art assessment machine which allows Physmed Therapist to assess our patients over all balance status, just like in a hospital setting. (This is not the Wii) A patient would simply perform a series of test and from the results we would be able to find out what specific muscle is weak in the body and how to train that muscle. Through our test we can see if the potential to have a fall comes from the right thigh, the lower back, the right side of the body or if it is something wrong with the patient’s sight. Assessment machine like the Balance master can be found in the hospital/rehabilitation setting for around $250,000.00 dollars. Physmed Balance Master is portable and starts at $18,500.00. The Balance Master is free to all patients and anyone who needs to be assessed. Physmed can send the assessment to the doctor and get the therapy order, if needed.

Nursing/Therapy Management:
    Nursing/Therapy meetings will be held every Wednesday in the Nursing/Therapy department with our Director Therapy/Nursing Coordinator & complete staff. Goals will be set for each patient for the week and the prior week’s results will be discussed. Physmed will monitor each patient while communicating with families, communities & staff on a daily basis to ensure that the circle of communication is not broken.

    Our team will discuss individual care plans, so that we can better assist your patient. Each Patient will have a case manager that will act as a contact person for family, physician, communities & staff.

Skilled Nursing:

    Physmed Skilled nursing is available 24/7! Our nursing staff will be there for our patients day or night, weekend or holiday to ensure that their needs are being met. Whether our patients need wound care, medication education or I.V Therapy, Physmed nursing staff will do whatever it takes to answer the call. Physmed staff will provide the Coagu –Check which can check PT/INR levels in two minutes having the results instantly. Choles-Tech can check the HDL, LDL and Triglyceride levels within 3minutes. Physmed also has the I-Stat (a mini lab) mobile bedside lab that can check Sodium, Potassium, Chloride, Ionized Calcium, Total Carbon Dioxide, BUN and Creatinine with results in 2 minutes. Physmed will provide all services free of charge!

    The following machines allow us to obtain blood values as follows:  

    The Coaguchek is used to measure the PT/INR by finger stick.  This test shows if the anticoagulants have reached and are maintaining therapeutic levels.  The Cholestec is used to measure cholesterol and lipid levels

    The I-Stat measures the basic metabolism such as  potassium, sodium, ionized calcium, glucose hemoglobin, hematacrit, & carbon dioxide levels.  All tests can be done at bedside and results obtained within less than 5 minutes.

    Wound Vacs-Our nurses have been trained in Wound Vacs; these machines are used on wounds to promote optimal healing.  The vac is small and portable, allowing the patient to continue with their normal routine. 

    Dieticians are available as needed for our patients with nutrition issues such as:  eating disorders due to surgery, disease processes, and problems with maintaining weight.

    Social Workers are available to help obtain special services, help patients fill out applications for services, short-term counseling with patients and their families.

      Skilled Nursing” Pneumonia Program”: One of the top five (DRG) as stated by Dr. Eric Rice, a hospitalist at Methodist Hospital in Omaha Nebraska, is Pneumonia. Physmed home health care has developed a strong pre/post care program for patients that have had Pneumonia or that have early signs of Pneumonia. Physmed will provide a specialized POC (plan of care) with Physical Therapy, Occupational Therapy, and Speech Therapy. Our Skilled Nursing will provided follow up lab work with the I-Stat labs within 2 minutes, CPT treatments per patient for assisting mucus expectorating, IV Antibiotics/Fluid. When our patients need us the most, Physmed will be there to help keep them out of the hospital so they can recuperate in their own home with people that love them.

      Skilled Nursing” Flu Program”: The season Flu has been known to cause mild illness and even death. Now with the threat of H1N1 Flu, Physmed is steeping up our efforts to help or communities, families and patients to prevent and recover from this potentially deadly virus. Physmed will monitor our patients daily starting with skilled nursing treatments to therapy evaluations and treatments. Physmed will record vital signs daily while administering IV Fluids/Medications. We will use our I-Stat lab machine to check-electrolytes, chemistries, coagulation, cardiac makers, and hematology and blood gases with more follow up work to follow. By combining Skilled Nursing treatments with Therapy treatments we will be able to help our patients recover at a faster more proficient rate.

       

      Depression Screening: When a patient comes to Physmed, we know that for most people this is a very trying time in their life and for their loved ones that are far away, it’s even harder. Physmed will use a 2 & 9 point Depression Screening to see how we can best help our patients to recover mentally and physically. For patients that need that extra encouragement and attention will be set up with a Volunteer Companion, that will help to guide our patients through this challenging time.

Physical Therapy:

    Our therapist will work on a daily basis with our patients to assist them as they strengthen not only their bodies, but also their minds as they strive to live an independent life. Our therapists know that it is every patient’s goal to be as independent as possible. Each physical therapy patient will have a customized POC (plan of care) to help achieve their ultimate goal of independence. If needed, Physmed will also keep a patient on a Special Program as long as needed to assure that the individual will maintain the same quality of life that is desired. Physmed Therapists will work with family and staff to educate them on new therapy skills and programs.

Laser &Light Therapy - which can treat the following:
    soft tissue injuries, tendonitis, sprains/strains, back and neck pain, repetitive strain injuries, carpal tunnel syndrome, chondromalacia patellae, muscle pain, arthritis, nerve pain, shingles, non healing wounds, fractures, bell’s palsy, fractures, bursitis skin problems and much more.

Occupational Therapy:
    Our therapists understand the challenges the body faces when it comes to moving around in one’s home. Our Occupational Therapist are dedicated to helping our patient find new ways to maneuver their body while sharpening old skills such as bathing, turning on the stove, reaching for that high cupboard and much more in the comfort of their home.

Speech Therapy:

    When it comes to communication, speech is something that many of us take for granted. A speech therapist is one of the few people who know the difficulties that patients face when the wall of communication has been broken. Our speech therapist will help with swallow studies as well with helping to educate patients on what they can do to help dilute supplements for easier swallowing. Whether a patient is recovering from something as serious as a stroke or just aspirating, our therapists will give our patients the courage to say, “Hello” again.

    Physmed will assist with providing DME to all patients as well as helping to get the patients DME qualified with Medicare/Medicaid.

Same Day Documentation:
    After each visit, Physmed employees will leave a full copy of our patient’s notes in his or her file or with the patient.Physmed now has a on line file system for all physicians and asssited living communities. Physmed will document everything that is done with our patients starting with a set of vitals for nursing to fully accessing from their mental state of mind to their physical abilities and their physical limitation. Physmed will set goals to be obtained for every nursing/therapy session to encourage our patient to reach their potential. Everything that we do for our patients will be documented daily and any changes in overall patient care will be sent directly to the patient’s primary physician.

Video/Photo Documentation:
    When it is needed Physmed therapist will document your residents/our patient progress for viewing purpose of the patient, family & staff members with access. We want every one involved to see the progress that is being made so that if the patient starts to regress we can even show them what they are capable of doing. Documentation for POA:
    If a power of attorney is involved, we will send monthly copies of the patients documented notes for them to review. Ex: We do this so that if Mr. Smith son lives in Texas and is not able to see how his father is doing at PHYSMED, he can simply check his monthly notes or view his video documents to see that his father is now walking 90 feet per day instead of 40 feet.

In-service for POA:
    Physmed will provide an in-service for all Power of Attorney’s (POA’S) who need to be educated on our services as well as the well being of the patient.

Physmed will provide in-services for Communities staff on a monthly/bi monthly period, depending on request from community management.

Individual Case Management:

    It is our goal to access and manage our patient’s physical and emotional needs at the highest level of care. Each patient will be accessed individually by his/her Nurse/Therapist so that their personal goals can be laid out and met over a 60 to 120 day period.

24 hour “On-call” Therapy:
    At Physmed, we understand that not all falls, therapy related issues or nursing issues happen between 9am and 5pm. We have a team of dedicated therapist willing and waiting at your disposal when needed. When you contact Physmed Home Health Care day or night, you will be met with a friendly “Hello “by either the Director or the Director of Nursing of our family owned company.

Volunteer Companionship Program:
    Physmed Inc, will provide a volunteer companion (when available) at NO EXTRA COST, to our patient as needed. We believe that the power of friendship and the ongoing encouragement of a companion will help our patients attain their physical/skilled goals. When Physmed service ends, our goal is that the companions’ friendship continues through the life of both our patient and volunteer. All volunteers know that this is a lifelong commitment and Physmed hopes it’s a life changing experience for all involved. Special Events for PHYSMED PATIENTS:
    One of our primary goals at Physmed is to bring smiles to our patient’s faces. Each month our family at Physmed tries to come up with some way to interact with our patients by creating fun events to do with them. At the beginning of the year we will help to ring in the New Year. In February we will hand out little Valentine’s Day Bags. In March we will help celebrate St. Patty’s day. In April, each patient receives an April flower to plant. In May, we hand out May Day baskets. In June we have a Spring Day with our patients. In July we will hand out Freedom Bags for the fourth of July. In August we will hand out Husker Game Day bags for the first game. In September we hand out fall baskets. In October we usually have Elvis or another Icon visit each patient and community for Halloween. In November we hand out little handmade turkeys which are made from local elementary schools. In December we have the big guy himself “Santa Clause” coming out to spread holiday cheer. Each patient will receive a photo with Santa and a candy cane.

    Each month that a patient is with Physmed we will try to do whatever we can with our patients to brighten their day, because we feel that that’s what we would want done for our own family member. All of these special activities are done free of charge and they are meant to bring a smile to our patients face, because when you choose Physmed, you’re not choosing to become a patient, your choosing to become part of our family.
